Procedure:

Requirements at the time of filing…
  • Five (5) letters of recommendation (only 2 are to be from relatives on either side of the family). These letters are to be addressed to JUDGE TED KLAMMER, and MUST contain the address of the writer. The letter should be sealed in its own envelope, and marked “personal and confidential.”
  • Appropriate paperwork (packet supplied by Probate Court).
  • The name, address and phone number of the agency in which you plan to use for the adoption.
  • Each and every person over 18 living in the household must have a Child Abuse Registry Check. Each petitioner must apply individually for this check.
A letter of request for an abuse inquiry along with
  • Two (2) forms of personal identification
OR
  • A notarized letter including date of birth and social security number should be sent to:

The responses will be mailed to you and should be given to the Lake County Probate Court when you file your petition. Please allow 3-6 weeks for the reply.

Please note: When you receive your hearing notice in the mail, you will also receive a form, which has, your Court appointed assessor’s name and phone number. YOU must contact the assessor as soon as possible to schedule a home visit. This responsibility is yours.

Petitioners should have the following available for the home visit by the assessor. Everyone who lives in the home MUST be at home at the time of the home visit.

  • State Required Financial Form
  • Medical form filled out for each petitioner
  • Have available copies of the following for verification:
    • marriage license
    • any divorce papers from previous marriages
    • your birth certificate
    • some type of photo identification (driver’s license or passport)

$450.00 deposit (This does not include cost of certified copies or certified mail that is sent to your agency and to INS. It also does not include post-placement assessments OR addendums your agency may require.)
