BIRTH CORRECTION
Procedure:
- The following guidelines should be followed by anyone desiring to register an unrecorded birth, or correct a birth certificate.
- A person may file an application to register a birth or correct a birth record in the Court:
- If he was born in Lake County
- If he is a resident of Lake County and was born in any other County in Ohio.
- A person who does not reside in Ohio must file his/her application in the Probate Court of the county where he/she was born.
- The application must be made on the form furnished by this Court. The application must be written in ink or typed. Your signature must be witnessed by a Deputy Clerk of the Probate Court.
- The following persons may be proper witnesses to appear in Court to testify for the applicant:
- Parents of applicant, doctor, or licensed midwife who attended at the time of birth
- If one of the above persons appear in court personally or has his deposition taken by a notary public, an Applicant can exhibit one of the three (3) records below, then this will be sufficient evidence for registration or correction of birth
- Family Bible
- Old Family Record showing date of Birth
- Baptismal Certificate showing date, place of birth
- If applicant cannot comply with the above, then the Court requires the applicant to produce at least
three (3) of the following items, at least six (6) years or more old:
- Certificate of Baptism;
- Family Bible, or old family record showing date or place of birth;
- Attending physician’s certificate of records;
- Testimony of a brother or sister at least six years older than the
applicant, or any relative who remembers the applicant’s birth;
- Hospital, nursing home or clinic records of birth;
- Confirmation certificate or entrance into church, or cradle roll, if age of
birth is given;
- Honorable Discharge from U.S. Army or Navy (Discharge or Official Rooster);
- School records certified or signed by school officials, which indicates date
of birth or age at given time;
- Insurance policies showing date and place of birth or a statement from the
representative of the company in which the applicant was insured at least six
years ago;
- Certified copy of allocation for marriage license;
- Certificate of birth or death of child of applicant;
- Old legal documents showing age or place of birth;
- Old newspaper clippings of birth, baby books, or community directory;
- Naturalization record of parents;
- Old employment or lodge records showing date or place of birth;
- Genealogical record;
- Any old records showing area or place of birth;
- Social Security application. This may be obtained by writing to:

(state your name, social security number, and ask for a copy of your application on their form SS-5.) NO FEE
- The applicant requesting a registration of birth must also produce a statement that there is no record of his birth, signed by the officer in charge of the records where such birth should have been registered.
- An applicant that requires a correction of birth, must produce a certified copy of the birth certificate that needs to be corrected.
