• All paperwork must be typed. Lake County Rule 57.
• Forms required at initial filing: original will and copy of death certificate along with:
• Form 1.0 Surviving Spouse, Next of Kin, Legatees and Devisees;
• Form 2.0 Application to Probate Will, if applicable;
• Form 3.0 Application for Appointment of Appraiser, whether an appraisal is required or not. An appraisal form is not required when the estate is opened for a wrongful death action only;
• Form 4.0 Application for Authority to Administer Estate.
• Consent to Auditor's Evaluation (If Real Estate).
