CIVIL COMMITMENT

Procedure:

• If individual is admitted to Laurelwood Hospital, the attending physician sends a letter to Judge Klammer requesting commitment.
• If individual is not hospitalized, a referral form and Pathways assessment is required.
• Judge Klammer must approve the Affidavit and related documents.
• The person who is being committed must be a Lake County resident or there must be a Judgment Entry transferring jurisdiction from his or her county of residence.
