REPORT FORMS

Procedure:

Guardians are required to visit their ward/wards on a bi-monthly basis, complete a guardian/volunteer visitor report form upon visit, and send the completed form to the Lake County Probate Court.

Also, guardians are required to complete annual guardian report concerning their ward/wards.
