WRONGFUL DEATH/INSOLVENCY

Procedure:

• Provide the following: application, narrative statement, proposed entry, and waivers from next-of-kin Fee due at time of filing. Application will be scheduled for hearing.
• Verification of receipt and deposit, and report of distribution are both due 30 days after entry is signed
